At your direction, your provider may submit the rebate request and explanation of benefits on your behalf. Please ensure you and your provider coordinate who will submit the rebate request.OrIf you use your pharmacy/prescription insurance and pay out-of-pocket for your medicine, and the pharmacy or provider is unable to process your Savings Program card for instant savings, you will need to submit a rebate request.To receive your rebate:Your healthcare provider will submit a rebate request (claim) to the insurance company within 365 days of your purchase. Depending on your insurance coverage, a co-pay may or may not be collected at the time of treatment.You and your provider will receive an explanation of benefits (EOB) statement from your health insurance company.You or your doctor will submit your EOB to the STELARA withMe Savings Program.The STELARA withMe Savings Program will review your EOB and issue a rebate based on your selected payment preference at the time of enrollment: to your provider, to a Virtual Payment Card, or to you by check if you have paid in full with another form of payment and submitted proof of payment.
